At first when I saw the ending I was like WTF!

Then I thought it was Tony that got whacked and like they said in the end of the second to last episode "You don't hear it coming". But after thinking about it, the perspective of the story was not Tony's. Throughout the series we saw things that Tony never saw happen or even knew about. So the Point of View was always ours "The viewer's". We didn't hear it coming and the song kinda says it all in that reguard "It goes on and on and on".

Just my $0.02 anyways

agree with gravyboat above...what made the last scene so jarring to me was the change in perspective
all of a sudden you are given a glimpse of the world as tony sees it
the life that tony created requires him to live in constant peril of arrest and/or attack
in other words, every time a "door" opens, he has to warily look up and see who's coming at him
by association, his family is imperiled by his own doing
