Who do i contact to make sure it doesn't happen again? I received a ballot for a relative that has been deceased for a fairly long time.

Vote for them.
You would expect that in the 21st century they could get this right. Once this election is over the entire process needs to be fixed. For now see if your county has a web-site where you can change the status to deceased.

Just did this, call the supervisor of elections in the county it came from and ask for the procedure to have a deceased person removed from the voter rolls. In Pinellas county it involves a photocopy of a death certificate and a note signed by a relative.

I received a ballot for an uncle who lived in Pennsylvania but died 4 years ago and one for my son who has lived in Pennsylvania for 8 years.

I live in the hood and the house that was on my property before they tore it down and built a new one was some sort of bonding house. Its been over three years since anyone could have lived there besides my wife and myself. I still get mail for 8 people who used to live at that address. I get all sorts of fun things, refund checks, tax bills, prescription meds from the VA, court papers. I have tried to send the stuff back saying 'no longer at address' but it just comes right back to me so I toss it. Sometimes I 'accidentally' open one of them. I have had 4 stimulus checks for the full amount, court papers saying due to covid the case is dismissed, more court papers stating that they skipped the trial data so they are calling the bail in, tax bills and everything in between. I am waiting to get 8 mail-in ballots.

its not a matter of "IF" they can get it right, its a matter of " do they want to" ! big difference!

Answered my own question. Looks like a mail in ballot is automatically sent to all voters on the registration list.

Good friend of mine received five unsolicited ballots at her house in NY. She is the only one of the five that actually live in that house, the other four left over 20 years ago when she bought the house from them. I would post pics but they have her name and address and if I edited them out, you could not tell what they were..
