Default Prop issue: Install failure or material failure?

Today while on the water I lost my prop on my two week old boat. I have never experienced anything like this and looking for input as to what could have caused it so it can be avoided in the future.

We did not hit anything thing.

Motor: Yamaha 250SHO
Prop: Mercury Bravo 1 LT
Prop was installed at the boat builder

Looks like the wrong spacer between the nut and hub was used. Probably put it in reverse and slid right off.

It did come off after being in reverse.

Wrong hub parts, specifically the aft spacer washer.
