Very sorry for you and your dog.

I am an "animal person" and own numerous dogs and horses. As such I have had to make this decision too many times - even put down my own horses a few times. It isn't easy to make the decision but if you keep the animal's interest first the decision is a lot easier. Keep in mind your dog doesn't know all the vet visits, surgeries, meds, etc are meant to help them. Sometimes the kindest thing you can do for them is make the decision to euthanize.

You might see if you can find a vet who will come to your place to do it. It is less stressful for the pet.
