Default Computer email spam hijacking questions

I hear about computer email hijacking all the time and from my understanding it's when a hacker uses your PC to send spam email without you knowing about it.

Does the hacker really use your PC to send the email or does he simply use your email address as the sender?

I ask this because if they can use your PC without you knowing about it to send email then 1. wouldn't the sent emails show up in your sent box and 2. if you don't use an email client on your PC (use a web based client instead like hotmail) is this still possible?

Freakin spammers are really getting out of control. They are sending 2x more spam now then last year and it's only getting worse.
