kingair, that actually happened at a summer camp in upstate ny many years ago. Officer unloading his service weapon and jacked the slide while finger on trigger.
Good news is that he was pointing the .45 down and away from any other guys next to him.
Bad news is that he was on the second floor of the barracks... And over the head area He shot one of his troops right off the commode!
It's possible the box simply wasn't fastened to the stud and when the plug was pulled (if it was pulled, maybe something fell on it) from the outlet, it pulled the box out.
I could go with that Rob, but what would cause the cover to shatter?
It would be interesting to know where the pieces of the broken cover were on the floor in relations to the wall? Where they just on the floor at the baseboard or were they scattered about on the floor spreading out from the wall?

If you've just settled for Dolly on the left, things would've been fine and the lamp would still work, too....

Joe, aside from the electrical box being blown out of the wall, was there "any" damage to the electrical box itself? I take it there is no signs of blackening of wires or anything?
The two plugs were out of the outlet; no signs of damage, fire or bending.
The cover's pieces extended several feet from the outlet in all directions.
The outlet looks like it was hit with a bullet; the top screw ripped out of the box and outlet has spiderweb cracks in the plastic.
NO signs of impact, No signs of fire or smoke. Sheetrock dust in the outlet box is still white...
The heavy bed was right in front of the outlet; a person couldn't get their hand in to pull the plugs out of the wall.

On a more serious note.
I vote that at some point either the cleaning ladies
decided to move the bed side ways
a heavy bed as this will easily move with the knees locked against it
it will only need a light side ways pressure on the plastic to
shatter it as described, and with that much drywall there would be
only a few threads of the screws holding the outlet in
and it will pop out of the box
